This happened to another player too. It appears to happen when you use a blank ammy, that has been forged up, but I have not had it happen with canned ammys with say wisdom or con on them. Now the ammy reappears when you leave the area, but it does cause the heart to flutter when it happens. It seems to happen only with the poison immunity quest.
